Today is a big day for updates! On XBOX (360, One), a major update has been released (Title Update 34 for 360, and Content Update 22 for XB1, respectively), and are now live! In addition, a new Story Mode-themed skin pack is available as well! As of this writing, it is available on XBOX systems, and should roll out to Playstation consoles of all stripes, as well as the WiiU, in the very near future.
Wait, did I say "Playstation and WiiU updates"? Yes indeed! All the changelogs are included below, with fixes in spoilers (there are so very many fixes)! Let's dive right in.

Updates on XBOX consoles should currently be live! Updates for Playstation (PS3, PS4, Vita) and the WiiU should be rolling out in the very near future.
